IMM 1017 received but with a wrong first name.

Subject: IMM 1017 received but with a wrong first name.
  I have received the medical form, but part of my last name was added to the first name.

Ex: Last Name is Van Heerden
First Name is Paul


In the form i have received they wrote my first name as Paul Heerden.

Anybody has a similar case or i´m the only one who´s soo lucky.

Dear Paul,
You should inform the CHC office of the mistake.Also inform the Doctor doing your medicals of the mistake.When sending the medical results he should put the name as it should appear.

Hi Paul,

Like Mike mentioned write a letter to the Consulate handling your case and tell them that a part of your name is missing. The DMP where you will get your medical done can change the name.

But I dont think this is a mistake. If your name is too long they leave out a part of it - generally the middle name. The same thing will happen when you get your PR card. Just to be on the safe side write a letter and see what they tell you.

Well as advised, go ahead with your medicals - name change can happen. Tell the doctor of the same so he can mention on the forms (take a passport for verification) - the doctor can inscribe passport no. on the form to identify.

That happened to me too, my daughter´s name was misspelled. Nicole spelled as NOCOLE, my then consultant told me to just inform the doctor about it and make sure that correction will be made on the medical form...go ahead with you medicals.

I did not send a fax to the immigration...I told my doctor and made sure he corrected the form...he wrote the correct name over the wrong one then signed beside the corrected name....just bring some identifications with you.
