  I have provide the following information with request

Date of Birth:
Country of Birth:
Approximate date that application was submitted:
Office to which application was sent (city, country)
Type of application:
File No:

After IA in (Jan 2005), I submited Medical, RPRF and Security checks documents in FEB 2005. It is now more than 40 weeks after IA.

What is right format or tone of composition of email, so that I will get status of my application.

Dear Sir/Madam:

I have these timelines, and its been this many weeks since my IA. Could you kindly please check my file#xxxxxxxx and let me know when I could possibly get a PPR, also please do let me know if there anything else which is required from me which might be holding up or delaying the PPR issual from your side. I received reply from CIC that, permanent residence will be reviewed for finalization early 2006.
