| Subject: Needed to attach my work permit for CAIPS request? |

Hi everyone!
I sent my CAIPS request on Oct. 4, 2005 by fax at (613) 957-6517, but haven´t received the note yet. I used the form below,

and ticked "Privacy Act" as I live in Vancouver with my work permit.
I sent this form only, but I guess I misunderstood the instruction
>"do not accompany it with a letter or the Treasury Board form.
>Only use this form, the necessary consents and the fee
>(if applicable) when making a request."
(1) I thought, with my Client ID, they are checking my visa status automatically, but I guess I should have attached a photocopy of my work permit, correct?

but if I resend my request by mail, which address should I send to?
