| Subject: Personal information error in the Medical form ! |

I submitted my immigration application to Buffalo on Nov 2004. I got my File No on Mar 2005. I just received the medical form. However, my birth date on the form is wrong!
What can I do now besides faxing to buffalo? can I do the medical exam now or wait for their response? Will they send me a new form? or I need to return the old one to them?

In my case, my daughter´s first name was misspelled...I did not inform CIC about it, I just informed the doctor about the error, he corrected the name and signed over it...however, yours is a birthday error..it might be a lot different...maybe you can try asking your DMP (doctor) if such error can be corrected on his/her end just like what was done on my daughter´s name...or you can email Buffalo and ask them what you need to do...good luck

You can have the Doctor mark the change on the current forms. This way the form will have the corrected date (hard copy). You can simultaneously point out the error to CIC via email/mail/fax and they will correct your file (soft copy).
I have had this happen to me and this is exactly what I did. No problems whatsoever.

my e-client status also use the wrong birth date, maybe I should ask the doctor and the Buffalo simultaneously.
My timeline is:
app: Nov 22, 2004
aor: Mar 5, 2005
IA & med: Jan 9, 2006

doctor took the photocopy of passport from us and corrected the date of birth on form
