What do I tick in that form: "privacy act (no fees) or something else?
Do I need to put "receive copy of the records" as well, or, CAIPS notes are different from these records?

As you are living in Canada and ordering your own CAIPS notes tick "Privacy Act(no fee)".
You need to "receive copy of the records".
