1) I got medicals and I am from California. I came to know that it is $410 per applicant for medicals. It is v costly for me. What can I do here? What is the usual cost in USA. Please advise me.
2) Also I am expecting a baby. So how to proceed on my spouse medicals. please tell me the process in detail.

Congratulations on IA first.
1) Wow, quite expensive, I´t 165 Can$ in Quebec now (130 examination + 35 X-ray), in fact the same price it used to be more then 1.5 years ago when we had our medicals.
Can not help you on that, rather some info.
2)About the pregnancy and X-ray, I believe you should consult some doctors on this matter especially. I remember it was discussed here last year(not exactly the answet on your question, just some info):

Hello Candice:
I paid $590 for a family of three here in california. I think if you are pregnant they won´t be able to take x-rays. so they will ask you to send it later.
