Medical Insurance Card?

Subject: Medical Insurance Card?

Hi,

I just want to land Ottawa and then leave for US for several months. I won´t be in Canada this year. Can I apply the Medical Insurance Card later when I come back Canada (maybe in the year of 2008)? Also does the card has to be renewed each year? Is there any fee for the medical card in Ontario? Do I have to apply in person, not by mail?
