Background Information Form

Subject: Background Information Form
  I received my IA in August and completed my medicals as well as sent additional documents (including Background Information form) by mid-September. Today I receive a letter that contains a Background information form for me to fill out. Can somebody explain what this means and is there anybody out there with the same situation.

It is just one page requesting addresses, personal history, military service, government jobs, organisations. The title on top of the page says Background Information Form in bold print.

I received the same form. The explanation on the top of the
form reads:"There are incomplete sections on your application form." In my case, I did not have anything to
report for those sections ( military service, etc). On my original application, I put N/A on the first box of each section. Apparently, this was not good enough for them.
So, this time I put NOT APPLICABLE on every single box of each sections. See if you received this form for the same reason.

Thats exactly the same reason except for me, this is the third time I am doing this. The first time was for the original applications. The second time was with my medicals when they asked for additional documents. Now, this will be the third time.
