Can an applicant not include her spouse?

Subject: Can an applicant not include her spouse?
  My sister is asking what if she doesn´t include her husband in her application now, for reasons that he still have some things to take care of for an indefinite period of time, will that affect their application (her son, 5 yrs old is going with her though) ?

Does the husband have to go through medical examination even if he´s not accompanying them?

Husband has to go through medicals including all dependents. He can accompany or not.

if spouse and kids do not do medical they cannot be sponsored in the future.
