Correction in medical forms

Subject: Correction in medical forms
  I´m on a work permit in Canada and I have applied through Buffalo.
I recently received the medical request along with my IA. I found a mistake on the form A for medicals. It states my DOB to be 23 June, while it should be 28 June instead. I sent a letter to the consulate in Buffalo to make the correction and send me a new form however, its been 4 weeks and I still haven´t heard anything from them. I was wondering if I should get my medicals done using the same form and get the date of birth changed later, or should I wait for a new form. Any advice will be greatly appreciated.

Hi there,

I had a problem with my medical forms too.
They mentioned my status as Single and did not send me my husband´s medical forms.

I wrote them couple of emails and all I got was an automated response.
However, when the agency thru´ whom I was filing wrote and email they replied within a week.
I got my IA on March 15th ´06 and they resent my forms in July first week.

Hope urs will be quicker...!!

Make sure you mention your filenumber in the subject line and write a very polite email.

I think it´s important to go ahead with the medicals and then correct later, as it´s not your mistake, and inasmuch as you´re not hearing from them. Good Luck. (I assume the records you sent them have the correct DOB)

Hi folks, I did my medicals in 14th May 2006 and recived in Ottawa 19th May. Can anyone tell me, when should I can get PPR? Any information please help. Thanks

Sal,

You can go ahead with you medical just mention the correction to your DMP (doctor) and ask him to initial/sign the corrected section.

That happened to me too (wrong spelling of name) and I did the same and no problem whatsoever.
