| Subject: Need help to fill Caips Form |

Hello guys,
I want a help from some experts regarding IMM5563B form (to order caips). I am a applicant for SW category in Bflo and wants to order Caips.

1. On the top of the form they ask for:
Requesters´ refference number and Public rights administration
file number.
Should I leave it blank? coz I just know my immigration file number and will write it down under "required information" heading.
2.I have a valid student visa in Canada. So which should be marked from "Access to information Act with $5 fee" or "Privacy Act with no fees"
3.Should I tick on "receive copies of the records" or "Examine copies of the records in a government office"
