Name Inconsistencies

Subject: Name Inconsistencies
  I am looking for people?s opinions, suggestions, and answers. Please feel free to respond even though I understand that some of the questions might be somewhat legal questions. For the sake of demonstration, I will use the following name for my questions: Alex Ben Chuck.

Alex ? being the first name
Ben Chuck ? being the last name

My birth certificate, foreign passport, and US permanent resident card show my name as first of (Alex) and last of (Ben Chuck).

Every other piece of document that I have besides the documents mentioned above, i.e. US drivers license, US military discharge papers, criminal record (1 minor conviction), etc. all have my name as first of (Alex), middle of (Ben), last of (Chuck).

I am required to apply for rehabilitation and then later as a skilled worker. My questions are the following;

1. Which name should I use on all Canadian applications moving forward?
2. Which name should I use on the US forms I have to fill out in order to send to the Canadian embassy, such as FBI, police certificates, finger-print based cards for pulling criminal history?
3. I am required to send in a copy of my US drivers license, should I try and get the name changed on my drivers license to reflect my last name as (Ben Chuck) before I send it a copy of it or should I send everything as is and has been ever since I came to the US as a teenager. I am reluctant to change everything to reflect my birth certificate, passport, and US permanent resident cards because I?m assuming there are some documents that simply cannot be changed, such as military discharge papers, and some others.

This is a tough one for me to decide. I appreciate any feedback.
