where i can get a form for PNP or where to apply for the

No PNP program in Ontario (in reply to: PNP Application)
Unfortunately, Ontario does not have a PNP program, so you have to apply under the federal program and forward your application to the Canadian Consulate in Buffalo. Good luck!
