What is the probability for a person who applied for PR with a clean background and medical and who is well above the pass mark to get denied the PR ?

Very low. Canada is very interested in skilled and educated immigrants to fill an expanding economy and maintain population growth. You´d have to have something very unusual in your background to be refused, given what you said. Your criminal background check also has to be flawless.
