can I postpone medical exam for PR

Subject: can I postpone medical exam for PR
  I just received the letter for medical exam. Does this mean I will get the PR approved around 6 months later?
As I will start my PhD in USA next year, I think the time is not enough for me to fulfill my PR obligation before I finish my PhD. For example, I do my medical exam in August, then I have to land by August next year, but by then, I just start my PhD, so it is no way for me to stay in Canada in the next 5 year.
Does anyone know if I can ask for an extention (say 1-2 years) for the medical exam?

if u want to wait 1-2 yrs, you might as well withdraw the application and apply again with buffalo. you need to do the medical exam within the timeframe stated or ask the consulate for an extension stating why you are unable to meet the request.
