| Subject: PC-1 form for Police certificate for foreigners |

Hi,
I am an Indian citizen and currently I am in US on L1 visa( work permit). To obtain police certificate from US , I am applying to FBI with my finger prints from Local Sheriff office.
I have following questions
1) Is it necessary to send PC-1 form as well to FBI
2) If yes for above then do I need to mention my addresses in USA and India as well or only US address will do.
Please advise urgently

Sorry forgot to mention that in Finger print form , there are few items like
Your OCA
FBI No
Armed Forces No (MNU)
Miscellaneous No (MNU)
Social security No(SOC)
few of these are not clear to me like OCA , I don´t know what to write there. I have filled my Passport number in Miscellaneous No and Not applicable in FBI No, Armed forces no Is that correct ?
Anybody knows what is OCA and what to write there
