Korean studying Ph.D. (physics) in the U.S. (applied with 68 points)
I didn´t take any English proficiency test (high school for 2 yrs, B.S. in 4 yrs, M.S. in 2yrs, 3rd year Ph.D.)
It seems like getting recomendation letters from your advisors certainly help your claims.
I didn´s use immigration lawyer (it´s a waste of money in my opinion).

(in reply to: PPR with 68 points (no IELTS)) Congratulations!
Well, I think you are one of the few fortunate ones. But just getting good recommendations is no enough. Your file being reviewed by the right officer also matters.
I went to English schools since age 2.5 Yrs. I´ve been speaking English for more than 27 Years, did my masters´ in the US, got references from US professors noting that I have excellent command on the language. Yet, I had to take IELTS.
