I lost my CPR form... how do I get a new one?

Subject: I lost my CPR form... how do I get a new one?
  After my wife received her PR card, we either lost or threw away her CPR form thinking it wasn´t that important. Today we went to exchange her US driver´s license at RAMQ (Quebec) and they couldn´t process it without the original CPR. They said there was a way to get a certified copy, but that it could take up to 6 months?

Anybody know anything about this?

I exchanged my licence with PR card as proof of residency,

You can try going to to some licence office in your town & see if it works

If not call CIC call center

apparently in quebec pr card isn´t enough. Need hte CPR.
