Manila office, "G5" Application form? common-law

Subject: Manila office, "G5" Application form? common-law
  When recieving our first response from the Manila office we recieved 3 papers. One requested 3 things:

- Police certs from New Zealand (she travelled there briefly)
- Translated Birth Certificate(we did it, but we need a professional)
- Original valid Passport

There was also a "Consent to disclosure of information" form for the New Zealand police.

Now, the third paper asked us to complete the "enclosed application form (G5)" and return it in 30 days. Unfortunately there is no form labelled G5. I´m inclined to think they are refering to the New Zealand police form, but they already asked for that on a seperate page and it is not labeled with G5 (nor is it really an "application" form). Has anyone every recieved this request before for a G5 form?

On a seperate note, from other experiences does it sound like we have been accepted without an interview? Or do they just want more information before deciding about an interiew?

Update:

I talked to the New Zealand police, and they have told me to fax the "Consent to disclosure" police form to New Zealand and they will send the information to Manila.

This leads me to believe there is some form that is missing, as they request that the G5 application form be send to Manila...

Quite confusing. Hopefully someone else has had the same request and knows what this G5 form is. Any info would be helpfull as we don´t want to miss anything
