| Subject: Info on Representative Change |
Hi!
Need some help on understanding whether we can change the Rep. and under what condition. Mine is too lousy. I understand that there is a Form 5476E to do that. Any suggestions Plez.

If your changing your representative the new authorized representative will explain it easier in person. Page 2 of the IMM 5476 has a place to remove the representative.
