Subject: Help me with form 5406E section A |

I am helping a friend who live here in Canada to fillout the forms to bring his wife to Canada, who is now living in Cuba.
I don´t understand why this form ask for Applicant name again, spouse, ex-espouse or ex-common-law partner.
Question:
Should I put my friend wife name in the first line?
Should I put my friend name, who is the sponsor in the second line?
After, why they ask to sign that there is not spouse,not spouse where, in other country?
what should I do,
Should I write in that case "not applicable"?

Your friend should fill out this form with the following informacion:
The applicant name again (who is Canadian Citizen or Resident Permanent), DOB, POB, Marital status and present Address.
The wife´s name again, DOB, POB, Marital status and present Address.
The same information for the parents.
Your friend don´t need to sign "Section A" (below Note 1) because he is married. If your friend were single, he should sign this section.
Complete Section B if your friend has any child. If he doesn´t have any child, just sign below Note 2.
Complete Section C if you have brothers or sisters.
Don´t forget to sign Section D. It´s mandatory.
