Subject: CAIPS notes
  Hello,

I know there are already some posts about ordering CAIPS notes, but I didn´t find a clear answer to my question:

I am sponsoring my husband and we live outside of Canada. I am going to get my father to order Caips notes for me. What do I fill in for Requester´s Reference Number and ATIP file number? Would I only have these if I´ve asked for CAIPS notes before? (I have not.)

I still don´t understand the difference between the PRIVACY ACT and the ACCESS TO INFORMATION ACT, except the ATIA is $5.00. Which one should I check?

Then, the request is being made by my father?
On the following page, the instructions say not to include a letter, but then below it says to start processing a request you need: A formal request and consents.

Is the formal request the form? What is the consent?

Hi Rosanna...

Okay... your dad will need to do the request in your name under the ACCESS TO INFORMATION ACT and he needs to send a check or money order for $5.00... you pay when you are requesting the information in someone else´s behave..

He doesn´t need to include a FORMAL REQUEST FORM the form itself is enough... but he needs to include a CONSENT LETTER from you authorizing him to get CAIPS for you... It does not need to be an original... you could email it or fax it to him... if you visit www.caipsnotes.com you can find a sample of this consent letter to guide you...(Step 3: consent letter)

When your dad fills out the form, he will check the first box under the ACCESS TO INFORMATION ACT... if you only want a copy of the records, check the next box if you want the record on a CD...

Then down below... REQUEST... you only need to check the box for CAIPS... the rest of the stuff is something else...

Just to be on the safe side... check that link I gave you before... they give step by step instructions on what to do... and later you can also use it to read you CAIPS and to find out what the codes mean...

I just have one more question, does my husband also need to fill out a consent form? (I am sponsoring him to come to Canada and I have been approved ad a sponsor)

Wait..so I think my DAD is doing the request on behalf of my husband..so my husband needs to give consent to my dad..

Well I guess it depend on WHOSE file is it?

Who is the applicant?

I am not very familiar with family sponsorship issues as I am a SW applicant... but I will assume that who ever is the person named as the applicant in the file is the person who needs to give the consent...

My husband is applying for permanent resideence, I am Canadian and we both live in Ecuador. I guess it´s his consent then. THANKS!
