| Subject: received IA ... some doubts |

I have received IA and had some doubts in the documents asked to submit. Can anyone provide some information on what to do....
1) I have been asked to provide "no record" or "certificate of good conduct" from "STATE OF NEW YORK".
I have already submitted FBI approved finger prints with my application. How do we take STATE OF NEW YORK certificate.
I am staying at Rochester, NY from last one and half year.
2) I have been asked to provide ORIGINAL of my W-2 income statement. I will need the original for my future references. Can I provide them a copy with notary attested.
