I finally arrived, and i´m in regina working under a TWP. My spouse flew in from Mexico, (was almost sent back, but told the truth and paid off). I have enough points to file for SINP. After I (or if I) receieve the SINP certificate, and after I file for FSW, does my spouse still have to go back to Mexico for his Medical? Or can I apply for an extension of his visit and have his medical done here in Canada since we´re already here?? It would be much easier if he stayed as he is staying at home taking care of the two little ones. three and four years old, they are legal to stay with me for the duration of my TWP at the moment.

Congratulation on getting your family together. Must be a relief.
As far as I know (and not 100% sure) medical can be done anywhere as long as the doctor is on the CIC approved list.
Someone else wants to confirm this please?

I found it, in the applying to change conditions or extend your stay in canada, it states that qualifies to apply within canada. So i made his medical appointment for next week. We can send his application to Vegreville. I am under a 2 year TWP. My NOC is a "B" skill level which qualifies him to apply from within Canada.
