Subject: Additional family information form

Hi everybody
After getting me CSQ, I am going to apply for federal process and I have a question regarding filling out the form IMM5406 "Additional family information". In fact everyone over 18 years of age need to fill out this form.

Obviously I will write my name as applicant and my wife as spouse in my form but will it be inverse on the forms of my wife.

Thanks in advance for any advice.
