On the standard fingerprinting form (FD-258) [obtained from http://www.fbi.gov/hq/cjisd/fprequest.htm], could you pls tell me what shall I fill under "YOUR NO. OCA" and "FBI NO FBI"?
Also, pls let me know if I need to fill in just the name, date & place of birth on this form or do I also need to fill in other info
{employer name, address, reason fingerprinted, citizenship, social security no, sex, race, ght, wgt, eyes, hair}?
If I do not have some info (like the "ARMED FORCES NO.", shall I leave it blank or write "N/A")?

If you dont have OCA or FBI No, write N/A. Write N/A for all information you do not have.
If this is for Canada immigration, you do not need to fill in employer information.
You do need to complete other information, including SS number, race, hight, etc.
