Completed by:
? the principal applicant
? spouse or common-law partner
? each dependent child over the age of 18 years
I?m the principle applicant and the form looks like for the principle applicant only????

when you fill the form your name will come first as principal applicant.
when your spouse fill the same form her name will befirst as principal applicant.
