IDENTITY AND CIVIL STATUS DOCUMENTS
provide birth certificates, baptismal certificates or other official document confirming relationship (INCLUDING ALL PAGES OF HOUSEHOLD REGISTRIES) for you and each of your family members..
what are the pages of household registries? english is not my first language so im having difficulty figuring this out.. can someone help me with this?
