Hi, i am a canadian citizen currently living in USA. I am applying my spouse who is living in the US with me. On the Appendix D - medical instructions, it mentioned that the form is used for application that will be processed OUTSIDE Canada. My question is where the results of the medical should be sent to, Canada? or the visa office in Buffalo, NY? Please answer ASAP as the medical appointment is tomorrow. Thank you very much.

The medical results are sent by the DMP (designated medical practitioner) to a central address in Ottawa. They are then logged in a system and matched to the applicant´s file number and the results are sent to the visa office processing the application. You do not need to mention anything about the visa post processing the file, all medical results are sent to Ottawa regardless of visa post. It can take several months for the medical results to be received by the visa post.

On the medical instructions form, there is a black "Visa office that will be processing the file", what should I fill in for this black? I am currently living in Massachusetts, USA. Should I fill "Buffalo, NY" as the visa office? Please advise asap. Thank you.

Yes, your processing visa office is Buffalo.

guys all medical results for the whole applicants send to ottawa or just for applicants live inside canada?

Jassy´s 100% correct, as you are living in Massachusetts your permanent residence application will be processed by the processing centre in Buffalo, NY.
Enter that on the form and/or inform the DMP of it when you go to have your medical performed. They´ll know what to do with that information.

I think Ottawa processes ALL medicals - both inside and outside.
