Subject: URGENT! ADVICE WANTED - MISSING MEDICAL FORMS
  Hi,

Having send my updated documents in Nov ´08 and then Jan 2009, I have been waiting for the medical. Finally got a letter today, but it turns out to be a reminder, telling me that I have 30 days to get the medicals done and send along with RPRF.

The problem is I never recd their letter (sent in Feb) asking me to get the medicals done. So now I have a 30 day deadline (and thanks to this letter reaching me 8 days late, I am left with 22 days) and no medical forms.

I have already sent an email to them about the matter using the London office webform. Does anyone have their fax no.?

Also would it be a good idea to send the RPRF and with it send a letter telling them about the missing letter and forms? Would that help, since if they don´t respond and the 22 day lapses, they will cancel and shut my file for good.

Please please help and guide me.

The first letter that you did not receive may have looked like this:

Failure to provide this information within the next thirty days will result in a decision being rendered in regards to your ability to meet the statutory requirements.

Before a decision made on this matter, you have the opportunity to provide any information you would like to be considered. Please write to this office within thirty (3) days of the date of this letter. If you do not, the decision about your application for permanent residence will be made based on the information on your file. If you not reply within 30 days and the decision is to refuse your application, there is no authority to re-examine or re-open this decision. If you wished any new information to be considered, you would to submit a new application, including new fees.

If you sent passport size photos in November 2008 and applied under Federal Skilled Worker Class without a legal representative, your response letter should be as followed;

This letter refers to my application for permanent residence from outside Canada on Federal Skilled Worker Class.

In previous correspondence, I was reminded that I require to have a medical examination. However, I did not receive the necessary forms, i,e, the IMM 1017E “Medical Report-Section A”. As a result, I have since NOT been able to arrange have my examinations completed.

Moreover, I have enclosed a money order for Right of Permanent Resident Fee.


Before a decision is made on my application for permanent residence, I would like to take the opportunity this information to be considered.
