| Subject: FBI and US State report for Canadian immigration |

Hi,
I studied for about 3 years in US back in 90´s (in Iowa). Came back after my studies and have been residing in Pakistan (Lahore) ever since.
I am in the process of applying for Canadian immigration and need to get the FBI clearance and also a police report from the state of Iowa.
I was able to find the required FBI form (http://www.fbi.gov/filelink.html?file=/hq/cjisd/pdf/fpcardb.pdf). However now I have no clue how to go about getting this form attested/finger-printed from a authorized person/agency in Lahore.
Can anyone help me or guide me in this regard. Also, is the above mentioned form the right one?
My 2nd and related question is how to get the police report from the state of Iowa? I have not been able to find anything so far.
