I applied PR sometime late 2005 and received a response letter from Visa Office - Manila that my application will be processed actively within 42 -46 months ( May - September 2009 )from received of application. Itīs October now and I have not heard from them todate. Should I make a follow up. Please advise

(in reply to: Is CIC office true to its words?) I applied in Manila first and after 4.5 YEARS OF WAITING (NO NEWS WHATSOEVER!), I got my papers transferred to Japan because I reside there (recently and fortunately the Tokyo office started processing immigration applications).
You have to know that Manila is one of the offices with the most backlog.
If I were you I´d follow up with a letter, although the probability of getting a reply is very slim. Anyway in the letter be as clear and straightforward as possible. For example, include a line saying "I paid the application processing fees and still received no news". Also, make sure to send it as registered mail.
