Subject: Questions about Ontario name change
  Currently I try to change my name, I am borned at China, however I can´t obtain my original birth certificate, they sent back the application and want me to provide "A copy of my record of landing" and "entry passport", there are no ways i can obtain my entry passport, I believed I lost it somehow during home moving, what can do now? I am Canadian citizen, resided in Ontario for almost 10 years.

1. Obtain your original birth certificate from China.


2. Apply for your record of landing

Applying for a certified true copy, correction, or replacement of an immigration document (IMM 5545)

Hi, thanks for replying to my post, I don´t have any relatives live at China right now, therefore I cannot obtain the the birth certificate from China. how about the entry passport, I can´t find it and do I have to replace that as well, thanks

Are you familiar with the embassy or consulate?

They issue family registrars or passports for their nationals.

For the lost passport, you may have to declare it is either stolen or lost.

And you may do some digging. There must be a place where a copy of your passport is still in archive.
