Subject: Address Quebec Health Card
  Last month I applied for a Quebec health card and gave an address in Montreal (an apartment) at which we were staying in order to receive the card.

As proof of my address in Montreal, the owner of the apartment issued me with a letter stating that he was letting me stay at the address for at least the first two months after my arrival in Montreal.

I was told by the official at the Quebec health office that I needed the letter from the owner signed by a commisioner of oaths with the presence of the owner of the apartment. I was given 45 days to submit the signed letter.

My problem is that the owner of the apartment is not in Canada and won´t return until after the 45 day period. Hence there is no way I can get the letter signed by a commissioner of oaths nor our health cards.

I would like to ask what would be the best thing to do in this situation. I could try and rent somewhere else if necessary just so we have a proper rental contract but this would be a great extra expense and we already have an apartment to stay in. If we do have to rent another property, will there be any problems due to the change in address from the one I orignally gave to the Quebec health office?

Hi,

If you have friends in Montreal you may change your address to theirs but then, you´ll need to provide their proof of lease. You may also provide the proof of lease of the person at whose apartment you´re staying at the mo.
