There is no re-entry visa. You either have a single entry visa that allows to enter Canada once, or actually several times if you travel to the US and you come back to Canada within the vadility of your permit to stay in Canada.
Or a multiple entry visa.
Either way, you have to apply from outside of Canada at the High Comission, Consulate or Canadian Embassy.

I think you´re asking about a travel document. (You´re a PR who is outside of Canada and wants to return to Canada but either does not have a PR card or has an expired PR card.) If so, the information is here:

i just want to know the requirements to get a multiple visa coz i´m planning to have a vacation in philippines on aug.2011 but my visa will expire on may 2011.i´m not yet a permanent resident here in canada but i have a valid work permit.could you please send me where to get the forms for that.thank you!

no such thing as re-entry visa...either you apply for multiple visa in seattle USA 3months ahead.of course lots of dollar and takes time...option 2 (the best way), as soon as you arrive manila go to the canadian embassy right away and apply for another entry visa.it takes few weeks or longer.follow all the instructions from the reception desk. be sure you have all the supporting travel papers from your employer and valid work permit. its been a week already since I submitted my application for a multiple entry visa,,I m just worried I sent my application thru LBC,,and without a returning prepaid envelop..I track it down they received it already.
I m just hoping that Air21 their courier will sent my passport back.

I just want to know what are the requirements to get a multiple visa because my visa was expired 2011 and I am planning to have a vacation this coming October 13 going to my country Philippines. I have my open work permit.
Can you help me send the application form of re entry visa..
