Good morning, thank you for reading my question.
I have recieve a client update form along with medical request. I´m trying to figure out the Work Activity part.
Am not employed but I do baby sitting Ocasionally. Do I need to put that on the form?
[b] If you were not working what it is you were doing in the occupation column. I start volunteering Oct, 09 to June,2010 then I satarted to volunteer at the same school from Feb,2011 to June, 2011
Should I state that on the form as well?

Simple answer is yes. The baby sitting, depending how often you are doing it I might leave out but the volunteering definitely needs to go on the form.
If there is a period where you did not volunteer work, make sure you put unemployed and the city you were living in at the time.
