| Subject: ???? Mistake is medical forms ???? |

Today I received letter from CIC about Medical Exam and Police certificate. But found mistake/wrong info (birth date and first name)printed on medical forms.
How should I correct them or notify them? Do I need to fax CIC corrections? Are they going to send me another forms? Will it affect on my application processing time?

Just fax them the correct name and date of birth along with your file number. That will do it. When taking the medical inform the DMP about the error and he/she will take of it. Hope that helps.
