Yes, you can get easily get TD visa for your wife from Islamabad or even from Karachi as Khi office also started giving non-immigrant visas. For TD Visa there is no condition for the spouse to be a canadian citizen or PR. Spouse is a dependant of TN Visa holder regardless her canadian status.

You cannot apply TD Visa from Pakistan unless you have TN visa entry and I-94 in hand. It means you need to come down to USA first on TN, work for a month so you will have paystub, bank statement, and job letter from your employer. Fillout online DS-160 form and send these papers to your wife. She will go with these papers, her passport and Nikah-nama (original and english both), wedding pics and video. They will give interview date and finally she will get TD visa on her passport.
Note: she is not canadian citizen unlike you so she will need a regular stamped visa on her passport.
If you would have H1 in your hand rather then TN then you can have her H4 being there in Pak and come together. Remember, TN can only be processed while entering into USA not in any consulate, whereas H1 is approved visa already.

forgot to mention one thing.
The above scenerio is for the spouse who is not canadian citizen. If she is and hold canadian passport then simply get a flight with her and both TN/TD will be processed at USA airport. Your passports should have at least 6 months time to expire. keep couple of US dollars in cash as they dont accept check or credit cards for TN/TD fee.
Also, I am not trying to scare you but keep arrangments to go back to Pak or Canada as TN might be rejected for number of reasons.

If your all paper work will be in order, it will take up to one month to get TD stamped.
File the online form, your wife goes to amex office in khi with confirmation letter, proof that she has valid TD visa, fee, pictures. They will give you around 2-weeks appointment. After the appointment, her pp will be ready in one week.
Yes, they know about TD visa. You also mention visa category in the online form you submitted. If you don´t find TD in the drop down list, choose other and specify TD visa.
Include the covering letter in the docs you wife will take at the appointment day, and explain TD visa in that letter.
