| Subject: Income tax report and assessment |

I have a pending h and c application. I was requested to do medicals almost 2 months ago, which i did. Just got a call from PRRA office Montreal asking me to submit my income tax report and assessment for the last 3 years.
I know what the assessment is but not sure what is the income tax report. Can someone please help.
Also how these reports affects h and c decisions.
Please note i did receive a first stage approval as yet.

T4 slips, pay stubs as income earner
Other stuff as self-employed
