| Subject: From Married Name to Maiden Name |
Hi!
Any idea on how do I change my married name to maiden name for the record? Got my divorce last year, currently an open permit, pr is on process and marital status updated frm married to divoced.

Service Ontario if living in Ontario and look for Office of the Registrar General.

Just to clarify some legal stuff: you are only asking to RESTORE your maiden name, not really changing as such!
