Hi,
1. Do we need to provide PCC and Medical test, ILT..
results while applying.

-U don´t need PCC or medical but yes, u need to show proof of ur English proficiency (like IELTS score or higher education from a school in English speaking country). Indian school or college degree is not considered as an equivalent.
2. What are the documents do we need to provide other then the one specified in checklist.

-U need to submit a copy of ur resume(if skilled worker class), any certifications or licenses u acquired in field of ur study, any appointment or job experience letter from previous employers, if u have an American degree or still in school, then a letter from ur student advisor stating ur status F-1, when u came to US, what is ur field of study, when u expect to finish ur degree and most important that u can legally stay in US on OPT for another 1 year period.
3. What is amount do i need to pay while applying.

-It´s CA$550 or equivalent in US$(Check on CIC website not the bank exchange rate). Better to pay in CA$(certified check from a US bank).

Hope this helps,
Aki
