Subject: Last minute application advice...please!
HI...my wife and I are almost ready to send our papers....she had her medical done yesterday so that is the last big thing to do except for paying fees. Getting our marriage certificate from Ontario was "fun" but we got it quicker with some help.

We have been going over the forms and adding N/A to many spaces and I have been elaborating on many topics so the application is growing.
I can´t wait to send the papers so I can stop second guessing myself.
Waiting will be easy......this changing and proof reading and writing something new and finding new evidence is terrible.

I am going to paperclip the extra sheets to the forms that they refer to....I hope that is procedure. I was wondering if I should attach the photos to the extra sheets which will be attached to the forms since the photos provide evidence that I mentioned in the extra sheets.

I will have all papers in order that they appear on the checklist. The checklist is divided into two sections...FORMS and SUPPORTING DOCUMENTS....I may have some supporting documents attached to forms...or is it better to separate everything?

If we are not using a representative we do not need to include that form, right? As well as the common law form?

One thing.......I see that all forms are to be originals except Sponsorship Agreement IMM 1344B..they say include a photocopy.....any reason for this?

Is there any other advice you could give us....we have been waiting for some time to get the application sent.....we do not want it returned because of something stupid........

Any advice would be greatly appreciated......

THANKS
RACHUNEK

By your post, I can see you have worked really hard to prepare your applications. I wish you good luck. My only advise is to make copies of each one of the documents, especialy the ones you don´t keep the originals: police certificates, medical exam form, etc. Buffalo is well-known for losing such documents. Please photocopy each one of them, besides it will help you to prepare for a potential interview.

Wish you good luck

make sure you send it either courier or by post with a track number so you have proof of send date. always handle documents with CIC this way.

Thanks for the advice...we are actually in Poland so we do not have to deal with Buffalo..... We have the up front medical done and the docotr filled out the form that is needed but he did not write anything in the last two boxes....

Name of office which IMM 5419 is sent to

Visa Office

We will include Appendix D and it has Visa office on it

I hope it is OK...I do want the application sent back due to a doctor mistake.....

This will be a bit late now but for anyone else I found sticking a ´post-it´ note to my evidence helpful as I could elaborate straight onto the evidence!
