| Subject: Question On Medical Forms |
Buffalo RPC only sent me medical Form A, but the letter mentioned that forms B,C,D,E are also required. Do DMPs (Designated Medical Practitioner) already have those forms B,C,D,E or I have to obtain them before I go to medicals?

the est of the form will be provided by your doctor, you don´t need to have them.
