| Subject: DMP forgot to sign the form |

My wife and I have just gone through all our forms to send...we did them step by step and had everything perfect...UNTIL we came to the last form on the checklist...
Medical form...our "brilliant" DMP forgot to sign it....my wife did not notice it when she was at the examination because the doctor was a chatterbox and kept talking and asking her questions. He stamped it and filled it out but no signature.....we will have delays due to the incompetence of a Canadian selected DMP.
Since we are in Poland he has already sent the form to Warsaw...we have not even sent our application to Mississauga yet. We were going to send it Monday but I am 100% positive that if we send it with the Medical form unsigned they will send it back. We must call our DMP Momday and let him know what "great" work he does.
ALL of you...be careful and make sure everything is done correctly....watch the DMP like a hawk.
This situation sucks but at least we caught it before we sent the application package to Mississauga. Hopefully the doctor can rectify this problem as quickly as possible.
Stay calm...be cool....double check everything

You are right.
Actually CIC mentions that on the medicals they send - that it is responsibility of the applicant to ensure all documents are filled out and signed.
Strange but true. You are not the first one. My medicals were claimed lost. I had to ask my clinic to fax copies and send by courier again. Cost me additional 30 bucks!!
So guys save time and money and after medicals ask when these will be sent out and if they have been signed. Most clinics will give you a copy for your records as after all this is a medical test you have paid for. The originals are ofcourse for CIC and cannot be given to you - but copies YES. So ask for these to make sure all is OK
