Subject: How to obtain a CAIPS notes?
  Hi All,

I plan to get a CAIPS notes for my immigration case. As I understand, if you can get help from a canadian citizen friend to fill out the request form plus my consent form, this should be enough.

Anyone can confirm this? And also people here can confirm if there is a need to send $5 ?
