Why wont you have your baby in Canada? No need to hassle with visa if you did that. I´m not sharon but I assume you´ll have to do family sponsorship and that was taking 6-9months.

Hi There! Don´t worry. If you and your wife is permnenet Residence of Canada, It will take only 10 working dyas to get Visa for your Child but if your wife deliver child in Canada, He or She will become Canadian Citizen By Birth, else your child have to wait (I assume) to get Canadian Citizenship.

How does it work in terms of cost? Does medical insurance cover pregnancy for new immigrants or they have to pay majority of the bills?

in terms of cost, it depends on the province where you are living. some provinces have a 90 day waiting period to qualify for medical coverage. once you are covered your hospital costs and doctor visits are covered. any medicines you are required to buy outside of hospital medications would be your responsibility.
