I applied for permanent residence in canada. Now I want to add my new born baby to my application. Which application form , should I have to fill up

Yes, I beleive there is a different application. I don´t know the name of top of my head, but you might have to reapply again.
When did you apply for your visa?
How long ago did you give birth?

This new family status will only help you to get your visa.

He born in April 2003 and I got the interview in the month of FEB 2004, so before FEB 2004 , I want to add my baby name.

This is a good point. You can add your baby´s name and that will actually help you with your papers. I don´t think it will speed up the process but it makes it concrete family case.

Immigration should be notified abou the change (in reply to: CHANGE IN FAMILY STATUS)
If you as a applicant get a child during the application process, the existing application must be updated to include that child.

For exact procedure contact Canadian immigration office (call them directly).
