Subject: Do I need a separate IMM5406 form for my wife?

Hi,
I am preparing the documents my self.We are applying as couple.I am the main applicant.I know the background form we need separate forms for me and her.but what about the Additionaly Family Form?Do i need a separate copy for her to put down her bros sistes etc? If so do I leave the applicant field blank in her form?
