| Subject: CAIPS form - need help |
I am getting a Canadian resident to submit the CAIPS form for me. But the form doesn´t seem to indicate where we need to mail the form to. Also what do we put down for Requester´s Reference # and Public Rights administration file number??? Can someone please advice?

Citizenship and Immigration Canada
Narono Building
360 Laurier Avenue West, 10th floor
Ottawa, ON
K1A 1L1
I just have receied the CAIPS. Person who sent it was Annik Quevillon, A/Senior Rights Administrator.
Concerning file/ref numbers, just indicate your file number in the application. You should have received it with the first letter from the CIC. It looks like B123456789.
