1/ depending where you are applying from, where you´re from , where you lived before and what class you´re applying in. Global averages are between 2 and 3 years for skilled workers, can be faster, can be slower.
2/ One year.

I think you have 1 year from the date of your medical exam to land.

Time of medical tests (in reply to: Timeframe to enter Canada after PR approval)
Thanks for the replies.
The time of medical tests is after submission of application or around the same time?

You must land before the expiration of your medical exam or the date at which your passport bearing the visa expires, whichever comes first. Since the medical exam is only good for one year (as Sharon has stated), this is the most relevant deadline for most people.

The medical exam is done before submission of the application for Family Class applicants, and after submission for most if not all others.

Specific information for your particular situation can be found at http://www.cic.gc.ca/english/index.html.

receiving letter of Application (in reply to: Timeframe to enter Canada after PR approval)
Hi,
I´m Sultana. I have submitted the application for Canada immigration at the last week of February 2011 from Bangladesh under skilled worker category. I found that my courier received by the authority. Also I found from my bank source that my bank draft ( fees) have been deposited . It is clearly proved that they got my application. Within 3 months they normally send receiving letter with tracking/file number. Unfortunately 4 months have been passed still I did not get any receiving letter or any tracking number. No email or mail. Shall I think that they rejected me or I will wait for the reply ? Do they have any time frame for sending acceptance/ receiving letter ? In this situation what will I do ?
